The Normandy Care Center works hand-in-hand with all of the area hospitals to ensure a seamless transition if you or your loved one is in need of short-term rehabilitation or long-term care. The admissions coordinator will communicate with the hospital discharge planner and your physician to identify your care needs prior to admission. Once you arrive, the interdisciplinary team will assist in developing your care plan and support you every step of the way until you are discharged home. If long-term care is identified, the team will continue to support your care needs each day; they are committed to ensuring the highest level of care and attention to support your physical, social and cognitive well-being.

Each admission is considered on an individual basis and depends on whether we can meet patient/resident needs.
